Captain R Seafood Salad order came with bugs Cleveland , Texas

On 12-16-10 I went into this restaurant where I ate most everday and placed my usual order which included a salad. The used to have the salad bar in the front where the customer could get their own salad but then they started having so much trouble with flies they shut it down and started keeping the salads in the back. When the girl brought my salad out I put my fork into it and something moved, I looked and it had what looked like knats or some kind of bugs in it. I told her and she reached down and took the salad up off the plate with her hands and asked me if I wanted another one and I told her that I did not think I wanted another one now after I had saw that. She then told me that I needed to leave that I ate there to much. I thought that was what a restaurant was for to eat in. also they (the girls) smoke everywhere in there not not just the smoke free area.
