family keeps charging me for a service that has been cancelled

i purchased a family talk $25 start up plan and a $19 phone from a t-mobile rep at walmart on 8/2/12 for a trial basis to see if i liked the service. i was told by the rep i could return it within 14 days and no penalty. before i got out of the parking lot i lost service, but i took it home and tried from there. i decided it was not for me and returned start up card and phone the following weekend. they gave me a full refund and said they would cancel the service. so a month and a half goes by and i recieve a letter in the mail saying i owe $77.72 to family mobile and my service has been suspended. i had never been sent a bill or a notice before this letter arrived. this for a phone i had for a week and talked on for less than 15 mins. went to walmart only to be told they could not help, try family mobile website. i tried family mobile website and was told i needed to go to walmart. seems to be a circle routine, the website is useless (almost seems on purpose confusing) and walmart reps appear to be clueless on how to solve the mistake they created or unwilling.
